FEE SCHEDULE

Initial Intake Session (90 min) = $175.00
Follow-Up Diagnostic Assessment (60 min) = $100.00
Individual Psychotherapy (30 min) = $50.00
Individual Psychotherapy (45 min) = $95.00
Individual Psychotherapy (60 min) = $115.00
Family Psychotherapies (60 min) = $115.00
Couple's Therapy (60 min) = $115.00
Child & Adolescent Level of Care Utilization System (CALOCUS) Assessment = $175.00
Specialized Assessments = $100 per hour. Amount of completed assessment to be agreed upon before the start of service. Prices vary based on the intensity of the assessment, required assessment tools, availability of information, and ancillary interviews.
Psychosexual Risk Assessment (PSRA) = $ 1,500.00 total. It is not covered by insurance or Medicaid.
Consultation / Training = $100.00 per hour. This service is for consultation with, and training for, other mental health providers, or organizations requesting such services.

PRIVATE INSURANCE PROVIDERS

MEDICAID

It is your responsibility to keep UCA, LLC. updated with any changes in your demographic information, and contact information.
Currently we are “in-network” with select private insurance providers. For those insurance plans not currently accepted please note all fees must be paid in full at the time of service, unless previously communicated and agreed upon. A Superbill will be provided to you, and you can file this with your insurance provider for any reimbursements.
The rates shown are not applicable to those clients with Medicaid, and this practice will adhere to the established fee schedule as identified by the SCDHHS regarding clients with Medicaid, or in coordination with their identified MCO. Should a client be changing MCOs, it is the responsibility of the client to inform UCA, LLC.
