We are able to accept United Healthcare (UHC) EAP, when the patient has their pre-authorization code.

Yes, at the counselor's discretion.

We have two locations, both within five miles of downtown Greenville, SC.


Which therapist you schedule with will dictate the location you go to.


Upon scheduling, you will be provided with an exact address of your counselor.

Yes. Our notes are written for the time frame that you attended therapy and are emailed to you after your therapy appointment  

(same day).

Our administrative staff will contact you via the email address you provided, usually within 2 business days.


If you submitted an appointment request, have not heard from us in this time frame, and you have checked your "spam" folder, reach out to us at: admin@ucassoc.org

Depending on the situation, the email response will be one of these:


1. Questions to clarify the information you provided and/or your specific counseling needs.

2. Intake paperwork for a therapist that can help and that has availability.

3. Referrals for other providers in the area that accept your insurance, in the event we are not able to help.

Why do I need to complete intake prior to making an appointment?

Intake must be completed prior to scheduling an appointment, as our intake contains forms we are legally required to have before beginning treatment, clinical information your counselor will need to review in advance of meeting with you, and required insurance and billing. 

 

If we were to schedule you prior to successful intake completion and there were items missing/incorrect, we would have to cancel or reschedule your appointment. That would be unfair to yourself, our therapist, and our other patients that could have used the time slot.

What is required to complete the intake forms?

Why do I need to complete intake prior to making an appointment?

The .pdf forms are electronically-editable, so no need to print.  It is advisable to use a computer, not a phone, to complete your forms.


Not all web browsers support .pdf fillable forms.  If you are unable to type on the forms, you will need Adobe Reader.  It is a free software, available for download here.


In the event you still cannot complete them electronically, you can print and scan or lay each document on a flat, well-lit surface, take a photo, and return them by replying to the email you received them on.

1. Download the forms attached to the email you received.  Note where the forms are being downloaded to on your computer.

2. Open each form and complete.

3. Save the form upon closing.

4. Reply to the email you received and attach your completed forms.

No, as we do not employ any in-office administrative staff and therapists do not have time in their schedules to receive them, nor the ability to process them.
